At the akinetic aneurysmatic apex a small gap of cardiac muscle as a hint for pseudoaneurysm or intramyocardial dissection were discussed (Fig. 1 a).
Left ventricular angiography demonstrated the large akinetic anterolateral wall aneurysm and a small contrast medium enhancement around the wall of the ventricle (Fig. 1 b) mimicking a covered ventricular rupture or intramyocardial dissection.
